  1. Best efforts have been made to accommodate requests. Please respect that we are volunteers, and we have in excess of 160 teams to place in a finite number of courts
  2. Only the Coach or Team Manager is allowed to submit change requests – If you are not the Coach or Team Manager, please do NOT complete this form
